Reunification Home Nurse position provides a combination of duties to ensure that the family has health and medical supports in place to support their health and well-being. The Nurse is responsible for determining on-going health and medical supports of the family to determine what is required for their health and well-being. Generally performs on-going physical assessments of families along with consultation between the Caseworker, Elders, Mental Health Therapist and family as well as other significant persons involved in the family’s life. Work with families to address their health issues to improve their physical health within a trauma informed framework and integrating a client centered and strength-based approach. The Nurse is to work in collaboration with Elders to ensure that the families have access to Siksika culture and traditions while on their healing journey.
